The NHS does not pay for some of the services we provide. Please note that the Practice can charge for NON-NHS Services.
These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions and some Travel vaccinations. We do not sign passport forms.
We require payment of our fee by BACs before we can process any requests/forms.
