How to register

Solva Surgery is a long established practice based in the beautiful harbour village of Solva.  Patients who wish to register with the practice will be expected to live within our practice area which comprises St Davids, Croesgoch, Trefin, Mathry, Llandeloy, Hayscastle, Roch and Newgale.

We currently have an open list. To register, please complete the registration form, which can be obtained from the practice.

We recommend that new patients undertake a health check with the practice nurse, who will take note of your medical history and current medication, which would otherwise not be available until your records are received from your previous practice.  This can often take several weeks.

Do I need to provide ID in order to register with a GP?

When registering with a GP, it is helpful if you can provide photo ID (passport or driving licence) AND at least one of the documents below:

  • A recent utility bill or proof of utility account, to show proof of address
  • NHS medical card
  • Birth certificate
  • For patients from overseas – a biometric residence permit/ a Home Office ‘travel document’ / or an ARC asylum seeker’s card
