Sick certificates

The rules for sickness certification changed in April 2010.  “Sick Notes” changed to “Fit Notes”.

Form SC1, “Self Certificate Claim Form for Sickness Benefit (including Invalidity Benefit)” for the self-employed, unemployed and those patients who have special medical treatment (dialysis, radiotherapy, chemotherapy) is available from the Surgery.

If you are an employee, your employers may ask you to complete their own self-certificate form or Form SC2 “Employer’s Statement of Sickness”.

You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days.

Sickness absence of less than 2 consecutive days – No sick note of any type required.

All claimants who have been sick for more than 7 days should arrange an appointment with their Doctor to obtain a sick note (Medical Certificate).
