Under the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018 (DPA 2018), you have rights as an individual that you can exercise in relation to the information we hold about you. You can exercise your information rights or make a request to access any personal information we hold about you at any time. Requests to access your information must be made in writing to the Practice Manager. You should provide enough information to identify yourself, and in regard to medical records, the period of time or specific treatment you want records for.
