- The core MSK Physiotherapy service in Pembrokeshire is piloting a method of digital self-referral to our services, which is currently replacing our paper-based self-referral form.
- We are directing all patients and professionals who have previously used the self-referral mechanism to the instructions attached below to complete the digital self-referral.
- If a patient is unable to access the digital platform due to technology restrictions they can contact the department on the number below for further options regarding self-referral.
- Following completion of the digital self-referral the department will receive this information and be in touch with the patient within the appropriate timescales.
